Pittsfield MA, 3 Sept 1903. The accident is recounted on pp.95-6 of 
"Trolley Car Treasury," picture and diagram included. Also (less 
thoroughly, surprisingly) on pp.30-1 of Connecticut Valley Chapter's 
Transportation Bulletin 79.
