The 1942 LVT accident had nothing to do with signals per se as the LVT was running two cars bunched together on the same schedule so the first car should have been flying following car flags, which the freight car operator apparently missed or forgot the operation of dbl headers was in effect! The PUC issued an order for LVT to run cars on a more frequent schedule and eliminate the dbl headers. The weird part of that accident was the fact the passenger car was in a similar accident on the C&LE, killing almost as many people.(12)   rogertrolley
