An operator tried to take a LRV through the PCC turnback loop at 
Washington Junction one evening. Seems the operator wasn't paying 
attention to the signal at the end of the platform. It was red 
indicating that a PCC had just been through the loop and the signal had 
not reset for mainline traffic. It entered the loop, made it about a 
quarter of the way and then derailed plowing through the center of the 
loop demolishing the dwarf signal ending up on the other side. I 
happened upon the scene the next morning, sadly without a camera. It 
took them all night to clean up the mess according to the fare collector 
at the junction.
