The terms would have easily varied from locale to locale.

The waste was minor compared to the amount of coal mined, otherwise it would 
not have been profitable.  There are some rather large waste piles in 
Pennsylvania that still need to be reclaimed, but they are small in 
comparison to the vast amount of coal extracted.

We still use lots of cinders on our township roads here in Armstrong County.

Most coal companies would not have moved the waste, hence the dumps.  Little 
money in it compared to coal and no environmental laws to require clean-ups.
