Ken-my understanding from several photo mags is that the following are 
restricted for photography(admittedly, probably justified):
1.nuclear installations;
2.military installations;
3.private businesses where trade secrets might be divulged
AND THAT'S IT!
The hospital thing w/ HIPPA relates to UNCONSENTED photos of patients or 
records, including unconsented use of non-anonymized radiologic images-if a 
patient or their surogate consents then it's legal. That is how such images 
make their way into medical texts, journals, and web sites so my colleagues 
and I can continue to learn.

I  agree w/ totally supporting transit police in their diligence in guarding 
against muggers, purse snatchers, panhandlers, fare evaders, exhibitionists, 
vandals, grafitti artists, those who smoke, eat and drink in transit space, 
those who put their filthy feet on seats, those who play radios loudly and 
offensively. We must not as photographers enter restricted space without 
permission-yards, track, etc. We must not interfere w/ boarding or egress of 
passengers. We must not use flash in tunnelswhere it could dazzle and blind 
an operator. In other words, we must be polite, respectful, and show good 
judgment. Do what our mothers taught us.
