A quick look at Pitt's library catalog turned up these two entries:

The report I have matches the description of the first item. The report you have doesn't seem to match the second item. But perhaps Pitt got a preprint of the report and PAT (and eventually you) got the spiffy printed-with-real-type bound-in-boards final edition?
